I understand that Salt Lake City Notary Services, LLC will keep the orginal copies of my documents for 3 months prior to disposing of them via shredder. I am capable of requesting original documents to be shipped via mail at an additional cost OR by providing a pre-paid mailing slip.

I can contact the state office to remove my agent or ask during my annual entity renewal NOT to renew with Salt Lake City Notary Services, LLC.

Notice of Annual Report Requirement: I understand this business must file an Annual Report to maintain 'active' status.

The first annual report is due one year from the effective date of the formation.

Salt Lake City Notary Services, LLC will contact you at least 1 month prior to the renewal to update the entity filing information and collect the renewal fees.
